SQL Server implementation of the Excalibur aggregate identity map store.

Setup

1. Create the table

    CONSTRAINT PK_IdentityMap PRIMARY KEY CLUSTERED (ExternalSystem, ExternalId, AggregateType),

2. Register the provider
